M & E / mHealth Consultant - Accra
The Community-based Hypertension Improvement Project (ComHIP) is a program designed to evaluate the impact of an innovative healthcare model on hypertension control and self-management in Ghana. The project works with its partners to develop hypertension awareness Within the community. organize screening and diagnosis facilities outside the health system. improve skills for Community nurses, and support self-managernent of hypertension by patients.
Position Summary
The successful candidate will lead the implementation of monitoring and evaluation plan for a community-based hypertension control program, manage the project's databaseand provide technical to the district health managerrwnt team to monitorirw of the projects activities. The consultant Will also provide support towards the development of national level roadmap for integration of Hypertension Services for the Ghana Health Service.

August 2019 to Decemeber 2019

Key Responsibilities
- Provide M&E technical direction to the district health management team (DHMT) prqect monitoring and evaluation (M&E) activities.
- Support the DHMT to develop and adopt a mobile data collection platform for hypertension services including development of indicators for tracking.
- Manage and track the project database and maintain the integrity of the CommCare platform.
- Liase with the project partners and providers to manage and track outputs of their services including the communications system database.
- Work on data analysis and lead reporting efforts of the project. Update the project database on a monthly basis.
- Contribute to the development of National roadmap for integration of hypertension services at the primary health facility level
- Contribute to project annual reporting
Minimum Requirements
- Bachelors degree in Social science, public health, biostatistics or related field.
- five years experience in monitoring and evaluation of health projects
- Proficiency in CommCare and Excel
- Familiarity of Ghana Health Service Structure and publlic health programming is an advantage
- Strong written and verbal communication skills including presentation of results and facilitation of discussion.
- Resourceful through technology, relationship building among several organizational teams/units and professional bodies to resolve problems
